Services are available to people who are uninsured, including males and females of all ages. Medicaid, Medicare, and some insurance plans are accepted.
$25.00 MINIMUM PAYMENT
INSURANCE CO-PAYS
The following multiple visit services are not subject to payment at time of service (these services must be paid fully in advance of first visit):
Dentures
Crowns
Bridges
Root Canals
Post Op visits
If you are uninsured, we use a Sliding Fee Scale. Depending upon your family size and household income, you may qualify for a lower fee from the Sliding Fee Scale. Sliding Fees are set as a percentage discount off the full fee. This percentage discount is applied toward all services eligible for the Sliding Fee. Sliding Fee percentages are set at the first appointment and are reviewed whenever there is a change in household income and/or at least annually.
