Sliding Scale Fee

To receive services at West Caldwell Health Council, you must be a registered patient. Payment for services is based on a sliding-fee scale according to family income. We accept most health insurance plans, including Medicaid and Medicare. To qualify for a sliding scale fee discount, please bring the following information with you to register at our clinics:

  1. Proof of Income, for every person living in the house over the age of 18. This may include:
    • a paycheck stub showing year-to-date income;
    • a copy of your signed tax form reporting your income,
    • a bank statement if you receive direct deposit and/or
    • a copy of any social security benefits you may be receiving
  2. Proof of Residence, for example, a bill addressed to yourself sent by mail.
  3. Evidence of health or dental insurance coverage, if any.
  4. A Picture I.D. card.
